To support your care and protect your information, we utilize secure electronic medical record systems and healthcare technology platforms, including but not limited to OptiMantra, Doximity, Doxy.me, secure telephone communications, secure messaging systems, and related practice management technologies.


By creating an account, registering, logging in, scheduling an appointment, participating in telehealth, or otherwise accessing or using the Services, you acknowledge that you have read and agree to the most current version of this Notice.

Emergency Care

Telehealth, office visits, or home visits are not appropriate for emergency situations.

If you experience chest pain, difficulty breathing, suicidal thoughts, severe bleeding, or any life-threatening condition, call 911 or go to the nearest emergency room immediately.

Providers may not respond immediately to portal messages or electronic communications.


Electronic communications, appointment requests, and telehealth platforms are not continuously monitored for emergency situations.


Commitment to Privacy

Enspire Beauty and Wellness, PLLC is legally required to maintain the confidentiality of your PHI and adhere to all applicable federal and state regulations governing its use and disclosure.


PHI includes identifiable information relating to your past, present, or future physical or mental health condition, healthcare services provided to you, payment for those services, and related demographic information.


As a primary care and telehealth practice, we collect personal and health information necessary to provide comprehensive medical care and operate our practice effectively. We are committed to protecting the privacy and confidentiality of our patients’ PHI and implementing safeguards to ensure its proper use and protection.

Limitations on Use by Minors

The Services offered by Enspire Beauty and Wellness, PLLC (“Enspire”) are intended for individuals who are at least eighteen (18) years of age, or the age of majority required under applicable state law. With healthcare providers participating in your treatment and, unless you object, individuals involved in your care or payment.


Business Associates

With contracted vendors who assist with electronic health records, billing, telehealth platforms, website hosting, communications, and administrative services. These entities are contractually required to safeguard PHI.


Legal and Regulatory Requirements

When required by law, subpoena, court order, public health authority, or regulatory agency.


Business Transfers

In connection with mergers or restructuring, subject to confidentiality protections.

We do not sell personal health information or permit independent third-party marketing use.


Uses or disclosures beyond treatment, payment, or healthcare operations generally require written authorization.


To Others Involved in Your Healthcare

Unless the patient objects, we may disclose their PHI to a family member, relative, close friend, or another individual identified by the patient, insofar as it relates to that person's involvement in the patient's healthcare. If the patient is unable to agree or object to such disclosure, we may share the PHI if we determine it is in the patient's best interest, based on professional judgment. Additionally, we may use or disclose the patient's PHI to notify or assist in notifying a family member, personal representative, or other individuals responsible for the patient's care, regarding the patient's general condition or death. If the patient is not present or able to agree, we will use professional judgment to disclose only the necessary information.


Other Permitted and Required Uses and Disclosures

We are permitted to use or disclose your PHI without your written authorization for the following purposes: as required by law, public health activities, health oversight, abuse or neglect reporting, Food and Drug Administration requirements, research, legal proceedings, law enforcement, coroner or funeral director purposes, organ donation, criminal activity, military or national security activities, worker's compensation, for inmates in correctional facilities, and as requested by the Department of Health and Human Services to determine compliance with the Privacy Rule.

  • Right to File a Complaint: You may file a complaint with Enspire Beauty and Wellness, PLLC or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. Filing a complaint will not affect the quality of care you receive.

Acceptance of Consent

Nature of Telehealth Services

Telehealth refers to the delivery of healthcare services using electronic communications when the provider and patient are not in the same physical location.


Telehealth may include:

• Live audio and/or video visits

• Telephone visits when appropriate

• Secure messaging

• Electronic transmission and review of medical records, images, or diagnostic results

• Treatment recommendations and prescriptions


Alternative care options, including in-person services, may be available when appropriate.


Benefits and Limitations

Telehealth may improve access to care, reduce travel time, increase convenience, and support continuity of care. Mental health services delivered via telehealth may improve coping and emotional well-being. However, limitations exist.


Risks may include:

• Technology failure or connectivity issues

• Delays in evaluation or treatment

• Inability to perform a full in-person physical examination

• Diagnostic inaccuracies due to incomplete information

• Medication reactions or treatment complications

• Security breaches despite reasonable safeguards

• False, inaccurate, or inconclusive laboratory results


No specific medical outcome or result is guaranteed. Your Provider may determine that telehealth is not appropriate and may recommend in-person evaluation or referral.


Privacy and Confidentiality

Enspire uses administrative, technical, and physical safeguards to protect your information in accordance with applicable law.


However, electronic communications are not completely secure. You are responsible for participating in visits from a private location and notifying your Provider if others are present.


Telehealth sessions will not be recorded without your written consent.


Your information may be disclosed as permitted by law for:

• Treatment, payment, and healthcare operations

• Mandatory reporting obligations

• Legal or regulatory requirements

Laboratory Services (If Applicable)


If laboratory testing is ordered:

• Testing may involve at-home collection kits or third-party laboratory facilities.

• Risks of specimen collection include bruising, bleeding, infection, or lightheadedness.

• Results may be false, inaccurate, or inconclusive.


Neither Enspire nor your Provider guarantees laboratory accuracy.


You are responsible for sharing results with your primary care provider and seeking appropriate follow-up care.


Laboratory testing is voluntary.

Where permitted by law, you elect to receive laboratory results directly upon completion.


Prescriptions and Financial Responsibility

Providers may prescribe medications when clinically appropriate. Certain medications, including controlled substances, may be subject to additional regulatory limitations.


You are responsible for all costs associated with services received, including prescriptions and laboratory services, unless otherwise agreed. You may choose your pharmacy when permitted.


Open Payments Notice

The federal Physician Payments Sunshine Act requires certain manufacturers of drugs, medical devices, and biologics to report payments or transfers of value over ten dollars ($10) made to physicians and teaching hospitals.

• You will provide accurate and complete medical information.

• You may withdraw consent to telehealth at any time by written notice, although Enspire does not provide in-person services.

• Withdrawal will not affect care already provided.
